Get your best skin yet in 2017
There are several things that we know for certain about the skin. One is that as we age, skin begins to thin due to a loss of collagen and elastin, resulting in fine lines and wrinkles. We also know that brown and red discoloration start to appear along with a dull, uneven complexion.
Throughout his years in practice, Amir M. Karam, M.D has seen firsthand that slowing down and even reversing these changes is absolutely possible. So what does it take? The answer is actually pretty simple (it takes constant stimulation in the skin). Here at Carmel Valley Facial Plastic Surgery, we know that through a combination of targeted in-office treatments and medical-grade skin care products, we can successfully prevent and reverse the signs of aging in the skin. Our complimentary anti-aging consults allow us to create a holistic, long-term maintenance plan that is unique to your age, genetics, and lifestyle. These consults are followed with regular evaluations and assessments to revise the plan and keep you in control, and looking young forever.
The beginning of a new year is always a great time to start fresh, especially with a new, proven regimen that keeps your skin looking bright and rejuvenated. To set up your own personalized, anti-aging consult call us today! We look forward to helping you start out 2017 with a bang!
A perfect blend between effective natural restoration, safety, and ultimately un-detectability
Aging is not fun for anyone. It starts in the 40s and accelerates with each passing year. The changes that result from aging universally have an unfavorable effect on appearance and self-image. The fact that they happen so quickly (over a period of 2-3 years) and so early, at a point when most of us still feel great and see ourselves as young and vibrant, makes it especially difficult to accept. Not just because of our vanity but mainly because it leaves us looking so different than the way we feel on the inside.
This is what I hear every day as our patients come in to discuss ways to help them restore their appearance or prevent aging from happening in the first place. Yes, both are possible. Let’s face it, wanting to look our best is a normal feeling. Not being comfortable with the fact that aging invariably leaves you looking tired, drawn, and less attractive is also a normal feeling that shouldn’t make you feel guilty. Believe me, you are not alone. What it comes down to is if you are not happy with how your youthful appearance is slipping away and there are procedures that can safely restore it without changing it, why not do it?
One major concern patients have is safety. No one wants to risk their life or risk having major complications in order to have a cosmetic procedure done. To avoid any anesthetic related risk, we perform our procedures under local anesthesia with sedation. This means you don’t have to be put under general anesthesia, and therefore, essentially all of the serious risks associated with surgery are eliminated. This is really important. All of our procedures are performed in our highly accredited (JCACHO/Medicare) surgical center, which allows us to train our nurses and surgical technicians in the highest standards of care and perform the procedures in a clean and monitored setting.
In addition, the technique that we use is perhaps the safest face and neck lift approach available. Tightening the loose muscles is paramount to an enduring and effective outcome. Doing this without risking facial nerves or poor scaring is a major advantage of our technique.
Another major reason that people shy away from face and neck lifts is fear. For many people, they fear that they will look like they have had something done. What does that mean? For those of us who have been horrified by the look of the man or woman sitting at a table in a restaurant who looks like they have just been through a wind tunnel, we assume that that is what happens when someone tries to go head to head with Mother Nature and get a facelift. The typical reaction is no thank you… I would rather look old. I don’t disagree. I hate that look as much as anyone… but I also know that this isn’t the result of the facelift but a result of who did their facelift and which technique they used.
If you knew that you could have a face and neck lift, get rid of the hanging jowls and turkey neck, and restore your facial shape back to its youthful heart-shaped configuration without anyone ever knowing because you don’t look even slightly different just refreshed, would you do it? Of course, why not?
So what makes our facelift so different?
Historically, the technique used for facelifts was to pull the face sideways toward the ears. This causes the face to stretch in an unnatural direction and leads to a pulled, unusual look. This also doesn’t do anything to lift the neck back into its original, youthful position. Unfortunately, this is still what you learn if you open just about any facelift textbook. Since most surgeons do what they were trained to do, this style of lift is still the most common technique used.
Our Short Incision Vertical Face and Neck Lift, starts with the concept that things need to go back where they came from. Simple as that. Following that principle, we simply restore the facial shape back to its youthful configuration by lifting it up in a vertical direction not horizontal. There will be no change in identity. The face will look the way it had prior to the onset of jawline and neck laxity. Who wouldn’t want that?
In summary, the Short Incision Vertical Face and Neck Lift is an outstanding way of restoring the facial shape back to its youthful, refreshed look, without the risk of changing your identity, looking like you have had work done or exposure to serious complications. This is a technique that I have performed over a thousand of times over the last 10 years and it never lets me or our patients down. Family members, spouses and friends frequently remark on how refreshed the person looks without ever being able to guess that they have just had a facelift. Emotionally, patients always express the boost in confidence that they experience as a result of looking their best, and more importantly, looking as youthful and fresh as they feel. That is perhaps the most exciting and rewarding part of our work- seeing the positive emotional impact facial rejuvenation creates.

When deciding to undergo facial rejuvenation or have a resurfacing laser or peel, pre-conditioning the skin can vastly improve procedure outcomes and lessen downtime. Here at Carmel Valley Facial Plastic Surgery, we pre-treat patients with the ZO Multi-Therapy Hydroquinone System (HQ Kit) and Retinol consistently, for approximately 4-6 weeks, before such procedures. The purpose is to help prevent any potential post-inflammatory skin discoloration as a result of the treatment. Also, skin that is healthy heals faster and allows for a more even treatment result.
In addition to being used as a pre-conditioning regimen for treatments, the kit is also perfect as a corrective boot camp for those looking to jumpstart their skincare regimen at home. For patients dealing with stubborn hyperpigmentation, uneven texture, and just overall sun damage, this system is exactly the extra push that their skin needs.
This amazing kit contains the following 5 travel size products: 2 forms of 4% hydroquinone (Melamin / Melamix), Glycogent (an exfoliation accelerator), Ossential Daily Power Defense (a potent DNA repair with antioxidants), and a SPF. This kit is designed to work best when combined with a retinol product in order to enhance overall product penetration.

In our modern world of social media and reality tv, we’re all too often exposed to aesthetic procedures gone awry—particularly when it comes to dermal fillers. We’ve all seen it, on our screens and on the streets: overly plumped lips and projected cheeks that leave the face looking disproportioned and unnatural. So often our reaction to such shocking results is to blame the procedure, rather than the techniques and hands behind it.
The truth is, fillers are a great option for treating volume loss and can create beautiful and completely natural-looking results. We know that volume loss doesn’t occur in only one area but across various regions of the face as we age. Dr. Karam’s solution, based on his unparalleled experience and expertise with fat transfer, is an innovative and comprehensive approach. Taking the entire face into consideration, he injects small amounts of filler into several different areas, creating a natural-looking, youthful, refreshed appearance.
Call today and let us help you schedule your very own comprehensive evaluation with Dr. Karam!
We are excited to announce that Dr. Karam has been invited to share his unparalleled knowledge and experience with his peers at the upcoming American Academy of Facial Plastic and Reconstructive Surgery meeting this month. He will give two facial rejuvenation lectures and serve as an expert on a panel discussing ways to avoid complications during facelift surgery.
The American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS) is the largest organization in the world devoted to facial plastic surgery. Founded in 1963, more than 2,500 facial plastic and reconstructive surgeons across the globe hold memberships to the esteemed association. Once a year the AAFPRS gathers for a four-day event to discuss innovations in the field and share their specialties with their colleagues. This year’s fall conference will take place in Nashville, Tennessee. The conference will be broken down into several series, each with panels and lectures on the industry’s most important and complex topics.
As an expert in the field specializing in facial rejuvenation and rhinoplasty, Dr. Karam has been asked to share his experience (which includes an astounding 350 fat transfers, 350 eye rejuvenations, 250 facelifts and neck lifts every year) and the innovative approaches he uses regularly as part of his Facial Rejuvenation practice. He will speak on his technique for achieving youthful eyes by way of Lateral Brow Lifting (browpexy) with Temporal Fascia Fixation and the advantages of perioral Erbium:YAG laser resurfacing as a way to remove the lines around the mouth.
Dr. Karam will also be on the Facelifting panel with three other revered facial plastic surgeons. In this groundbreaking panel, a moderator will probe Dr. Karam and his peers with in-depth questions, prompting them to share their facelift style and techniques geared toward ways of avoiding complications. At the terminus of the panel, attendees will have the opportunity to ask their own questions of the experienced surgeons.
The eyes are the window to our soul and the part of our face that most people are looking at during day-to-day conversation. Age affects our eyes in a variety of ways. Along with volume loss and sagging, that is best addressed with a combination of surgical and non-surgical treatments, we also begin to see changes in the texture and thickness of the skin around the eyes. With consistent use of medical- grade eye creams containing retinol and growth factors, we can prevent and reverse aging by thickening the skin and smoothing the texture around the eyes.
Another common age-related concern is thinning of eyelashes. Many of us try to disguise this problem by using false eyelashes or eyelash extensions. Why are these options not ideal? They can often be uncomfortable and can result in damage to your natural lashes. Latisse is an amazing alternative for those of us wanting to accentuate our eyes and keep them looking young and fresh. Latisse promotes growth of your natural lashes making them longer, fuller, and in some cases darker, in only 8 to 16 weeks.

Say Goodbye to Tired Eyes with Dr. Karam’s Comprehensive Eye Rejuvenation
Your eyes are the first thing people see when they look at your face; an immediate impression is formed about your age, level of energy and fatigue. Youthful eyes sparkle and look refreshed, but with aging comes a combination of changes that result in a less rejuvenated, more fatigued appearance. These changes typically start in the mid-to-late 30s and progress rapidly throughout the 40s and 50s.
Dr. Karam’s innovative approach to eye rejuvenation involves a comprehensive assessment of the aging-related changes around the eyes and a treatment plan that includes a variety of non-surgical and conservative surgical modalities that, when combined, truly rejuvenate the eyes with minimal risk and downtime. This approach leaves the eyes looking revitalized without changing their shape or function.
The three components of aging eyes and our unique approach to treating them:
Botox is used to decrease the line between the eyebrows (the 11’s) and the lines on the outer portion of the eyes (crow’s feet). The thinning, crepey skin can be resurfaced with either a Jessner’s 35% TCA chemical peel or Er:Yag laser resurfacing.
Medical-grade eye creams and Latisse are also used to hydrate the skin, improve dark circles, and generate longer, thicker, and more youthful looking eyelashes.
Fat Transfer is an important and highly emphasized part of our eye rejuvenation philosophy. This is a treatment that complements our surgical procedures and is done with essentially every rejuvenation case (approximately 350 cases per year). Adding volume is the most effective and important way to rejuvenate the eyes. Without it, the skin and sagging may look better, but the eyes will never have the refreshed and youthful look.
When excess skin is present, Dr. Karam uses one or a combination of the following conservative surgical procedures to remove it:
If you are tired of your tired eyes, Dr. Karam’s comprehensive eye rejuvenation is the solution. Schedule your consultation today and receive a completely customized plan that will leave you looking more refreshed and youthful!
Introducing our new Anti-Aging Program
Whether you’ve had surgery and want to maintain your results, or just focused on prevention, Carmel Valley Facial Plastic Surgery has the most advanced and innovative plan for you. Our prevention and maintenance philosophy is based on Dr. Karam’s twenty years of experience understanding how the face ages and discovering ways to stop it.
The concept can be best summarized as “active management”. This is not the same as going in for treatments from time to time without a specific aim or purpose. That approach rarely leads to meaningful results and often leads to a waste of time and money. At Carmel Valley Facial Plastic Surgery, we create a holistic, long-term management plan that is unique to your age, genetics, and lifestyle. We combine at home medical-grade skin care with regular in-office treatments (both surgical and non-surgical) to keep collagen production on an upward trajectory. By working against the natural decline of collagen, not only can we stop the signs of facial aging before they appear but also reverse those signs that are already evident. These components are coupled with regular evaluations and assessments to revise the plan and keep you in control and looking young forever.
For those interested in looking younger as you age, this is exactly the plan for you. To learn more, schedule your personalized anti-aging consultation today and begin your journey towards a younger-looking you!
What is happening to our skin as we age? One of the most significant signs of skin aging is a loss of volume. This loss of volume leads to hollowness, fine lines, wrinkles and ultimately sagging skin. Why does this happen? Simply stated, a loss of collagen. In our youth, collagen supports the underlying structure of the skin keeping it smooth, plump, and vibrant. It is continually being replenished as a normal part of healthy skin function and healing. Unfortunately, as we age, the production of new collagen also begins to slow down. So, not only are we losing the collagen we have, we are producing less as we age.
What can we do to preserve our collagen and how do we influence more collagen production within our skin? Growth factors are an amazing category of medical-grade, topical skincare products. Human derived growth factors dramatically influence collagen production in our skin. Long term use of growth factors will improve skin texture, elasticity and volume. Growth factors are recommended topically on the skin twice per day and the results are cumulative over time. With consistent use of growth factors for at least three to six months, you will definitely see the notable impact that they have on the quality of your skin.
In order to get the most out of your growth factors, it is paramount that you also incorporate skin treatments as part of your anti-aging plan. When we perform light treatments like our Forever Young BBL or skin resurfacing treatments like chemical peels, hydrafacials and laser procedures, new collagen is always formed as part of the skin’s healing process. The perfect marriage is to have regular treatments in order to upregulate the production of collagen and use growth factors to maintain and improve upon those results. In essence, the skin can continue to improve and look better over time rather than continue to age.
Call today to learn more about growth factors and schedule a complimentary anti-aging consultation with one of our aesthetic experts!
There are a series of natural and predictable repercussions that come with aging. As you mature, your skin’s production of collagen and elastin decrease, leading to fine lines, wrinkles and sagging. Years of exposure to the sun result in increased redness and brown spots. The good news? It’s all preventable, and even reversible, with the right treatments and products.
It’s incredibly important to invest in medical grade products, as opposed to over-the-counter, which are essentially unregulated and not held to any true standards. Equally as important are regular in-office treatments which have proved to be essential in preventing aging and maintaining your best results. Here at Carmel Valley Facial Plastic Surgery, we create personalized plans, combining these two elements, along with regular evaluations and assessments to revise the plan and keep you in control and looking forever young!

The importance of Medical Grade VS Over-the-Counter Skincare Products
There are literally thousands of skincare products for consumers to choose from. It is a billion dollar industry! Did you know there are about 7 or 8 “Parent” companies that own the majority of popular over the counter (OTC) brands available on the market today? What are OTC products and how do they differ from their Medical Grade counter parts? How do we know which products will produce the best results and actively address our skin concerns? With questionable advertising, outrageous claims, and airbrushed teenagers modeling skin creams for mature women, it’s challenging to know whether or not the information we are receiving is legitimate.
Let’s start with Medical Grade products. These products can only be purchased from a licensed doctor. They have the highest quality and concentration of active ingredients. These products have technologically advanced delivery systems to ensure product penetration deep into the skin. The efficacy claims of these product lines have to be backed by clinical studies and rigorous tests to prove they actually work. These are the only products that will significantly influence changes in skin biology and improve the overall health of the skin. It stands to reason why medical grade is the ONLY way to go when selecting your personalized skincare.
OTC (over- the-counter) products, on the other hand, are found in drug stores, spas, salons, 5 star hotels, high end department stores and anywhere else where a doctor’s license is not required to carry or sell them. Since most OTC products are owned by a monopoly of parent companies like Estee Lauder, you could spend $40.00(on Clinique, owned by Estee Lauder) or $400.00(on Crème De La Mer, also owned by Estee Lauder) and essentially see the same results… zero! There are no required standards on the efficacy of these products. They are packed with irritating perfumes, petrolatum (mineral oil) and inactive ingredients. These products are usually sold by sales experts, not skincare experts and in the simplest terms….. OTC products do NOT work no matter how expensive or beautiful the packaging is. Don’t be fooled by strategic ads that over promise and never deliver.
The reality is results come from long term plans that include corrective treatments and an at-home medical grade skin care regimen. If you maintain a commitment to this plan, your skin will grow healthier and more youthful year after year. When choosing the best Medical Grade products for your skin, trust the experts. Carmel Valley Facial Plastic Surgery is one of the few practices to take this comprehensive approach to slowing down the aging process. We are excited to offer skin rejuvenation consults as part of our Passport to Skin Rejuvenation Program. We design a customized plan for you to both restore and maintain the quality of your skin. Call and schedule a consult with one of our skin care experts today!
Just as the skin on the face begins to show visible signs of aging, such as sagging and wrinkling, so do the earlobes. With age and the pull of gravity, our earlobes begin to deflate and elongate over time. For many women, this process is exacerbated by a lifetime of wearing heavy earrings. Oftentimes, patients who deal with this problem are not aware that such a quick, simple solution is available.
At Carmel Valley Facial Plastic Surgery, we always support a comprehensive approach when it comes to facial rejuvenation. If you invest in a comprehensive rejuvenation program , but your earlobes remain drooped and wrinkled, they will look out of place and non-harmonious with the rest of the face.
Earlobe Rejuvenation is a quick and long-lasting procedure that requires zero downtime—you can even slip your earrings back in directly following the procedure! Using Juvederm, a hyaluronic acid filler, we can revolumize and restore earlobes, returning them to their youthful length and plumpness. Even the most severe cases of earlobe deflation can be alleviated at Carmel Valley Facial Plastic Surgery using a combination of fillers and surgical earlobe reduction.
With such an easy answer for aging earlobes, there’s no reason to hide them behind your hair. Open your ears to the world with Earlobe Rejuvenation at Carmel Valley Facial Plastic surgery!
Did you know that UV radiation accounts for about 75% of skin aging?
With summer here, it is important that you know how to protect your skin from the sun. UV radiation targets the DNA of the skin causing brown pigmentation, capillary damage, collagen loss, the breakdown of elastin fibers and skin cancer.
What is the best kind of sunscreen and what should you be looking for when selecting your sun protection?
With so many options and misleading information on the market, choosing a sunscreen can be very overwhelming. It is important to become educated and understand how different sunscreens work so that you can make the best selection for your skin.
There are many sunscreens that have Zinc and/or Titanium but what is most important is the percentage of these ingredients. If you do not see 7% or higher Zinc Oxide and / or Titanium Dioxide in the active ingredient list, don’t buy it. If the percentage is not above 7%, you are not getting Broad Spectrum protection from UV radiation. The higher the percentage, the better protection you will have.
Don’t be fooled by high SPF numbers on the front of your sunscreen as they only apply to UVB protection or how long you can stay in the sun without burning.
Today Zinc/Titanium based products are more cosmetically elegant than they have been in the past. Many even have a tint to make them blend better with the skin. Here at Carmel Valley Facial Plastic Surgery, a favorite is the Elta Physical ‘tinted’ sunscreen. It is non-comedogenic, cosmetically elegant, water-resistant and has no chemical sunscreen.
