Smoker's lines, the fine vertical creases that form above the upper lip, show up in patients who have never smoked. The term stuck because chronic smokers develop them aggressively, from years of repeatedly pursing their lips combined with tobacco's well-documented effects on skin aging. But the mechanism is universal. Aging changes the quality and thickness of the skin in this area, and the muscle that rings the lips keeps contracting with every movement you make.

The Anatomy Behind the Lines

The area above the upper lip is called the white lip. The skin there is relatively thick compared to most of the face, which directly affects which treatments will be effective. As we age, several changes happen simultaneously in this zone:

  • Collagen and elastin decline: These proteins give skin its resilience and snap-back ability. As they diminish, the skin becomes more prone to folding and creasing.
  • Fat loss beneath the skin: The subcutaneous layer thins with age, removing the cushion that once kept this area smooth.
  • Constant muscle activity: The orbicularis muscle rings the mouth and contracts every time you speak, sip, purse your lips, or kiss. It never stops.

Together, those changes create the conditions where vertical lines form. Smokers accelerate all three, which is why the lines appear on them so reliably. But the progression is universal.

From Dynamic Lines to Static Ones

Early on, these lines are dynamic: they appear during expression and vanish when the face relaxes. As collagen and elastin continue to diminish, those temporary folds stop disappearing. Eventually they're visible even at rest. Once that happens, what works as a treatment changes considerably.

Matching the Treatment to the Stage

For dynamic lines, a small amount of BOTOX® Cosmetic on each side of the upper lip can soften the orbicularis muscle just enough to prevent those folds from becoming permanent. Dosing matters more here than almost anywhere else on the face. Too much flattens the smile and noticeably weakens lip movement. My wife Neda has received one to two units per side for years and has no deep lines in this area. Done right, the treatment is imperceptible.

Mild static lines respond somewhat to filler, but technique is everything. Injecting directly beneath a line creates mounds on either side that can make the crease look even deeper, and repeated injections over time can produce a wavy distortion that's very difficult to reverse. A small amount, placed correctly, can help, but I caution strongly against overuse here.

For moderate to severe static lines, laser resurfacing is the most reliable option. The principle is direct: remove the skin to the base of the wrinkle, and the line can effectively be erased. Erbium YAG is precise and generates less heat than CO2 lasers or deep chemical peels, which substantially lowers the risk of hypopigmentation, the lasting skin lightening that can be difficult to correct. Recovery runs about two to three weeks. Fractional lasers and energy devices like radiofrequency or microneedling can improve skin quality, but they don't remove tissue, and tissue removal is what an etched static wrinkle requires.

The Earlier You Act, the More You Can Correct

The earlier you prioritize your skin, the better you can preserve its quality over time. Daily sunscreen, vitamin C, retinoids, and minimizing repetitive lip movements, such as drinking through a straw, can help reduce the formation of vertical lip lines while supporting healthier, more resilient skin. Skincare and structural aging are separate processes; skincare improves skin quality but does not correct sagging.
