Twenty years of performing facial rejuvenation surgery taught me that the best outcomes start with early intervention, before aging becomes established. That insight is what led me to develop Vertical Prevent®. I'm Dr. Amir Karam, a double board-certified facial plastic surgeon in San Diego with over 7,000 natural outcomes. Below are the questions patients ask me most.

Is Vertical Prevent a Facelift?

Yes. Vertical Prevent® is my proprietary facelift procedure designed for patients with early signs of facial aging who want to preserve their natural appearance before more advanced changes occur. Rather than waiting until significant skin laxity and tissue descent develop, Vertical Prevent® restores the underlying ligaments and fascia using a vertical lifting approach, allowing the skin to re-drape naturally. It addresses the outer brow, midface, jawline, and neck together to maintain facial harmony with subtle, natural-looking results.

Who Should Consider Vertical Prevent?

Most patients who benefit from this procedure are in their late 30s or early 40s, though anatomy matters more than age. The early signs I look for include:

  • Outer brow: Subtle descent creating a heavier, more tired look around the eyes
  • Midface: Cheeks that have softened and lost their projection
  • Jawline: Early laxity where the contour used to be crisp
  • Neck: Mild laxity, when present but not yet advanced

If your neck is still relatively unchanged and you're bothered by what you see in photographs, this is the right conversation to have.

How Does It Compare to Vertical Restore?

Both procedures reposition deeper facial structures vertically. The difference is scope. Vertical Restore® is my comprehensive approach for patients with established change across the outer brow, midface, jawline, and neck. Vertical Prevent addresses those same zones, earlier in the process, without the full reconstruction that Vertical Restore requires. The anatomy in front of me tells me which one is right.

What Does Recovery Look Like, and What Results Can I Expect?

Surgery takes two to three hours under local anesthesia with IV sedation. Most patients return to daily activities within 10 to 14 days, with swelling continuing to resolve over the following weeks. Every procedure is performed personally by me, without a fellow or physician assistant.

Because the correction is structural, results settle naturally. Patients tend to describe the outcome as looking more rested and more like themselves at an earlier age.

How Do I Know If I'm Ready, or If I Should Wait?

This is the question I spend the most time on in consultation, because the honest answer requires looking at the anatomy, not relying on a patient's self-assessment. Most people either underestimate what's happening or talk themselves out of acting because they feel they haven't aged enough. Neither instinct is a reliable guide.

What I look for is structural change, not severity. If the ligaments are beginning to loosen and the soft tissue is starting to descend, earlier correction yields a better result than waiting. Skin elasticity declines over time, which means tissue responds more predictably at 40 than it will at 50. Not everyone who comes in at 38 is ready, and some patients in their late 40s are still good candidates. A consultation gives me the information I need to tell you honestly which side of that line you're on.

20+ Years of Facial Rejuvenation Experience, Starting With Your Consultation

Two decades of working exclusively on the face has shown me that timing matters as much as technique. Most people who are right for Vertical Prevent sense it before they can name it. If you're noticing early changes and want an honest assessment of where you stand, schedule a consultation. We'll look at your anatomy together and determine what's right for you.
